The issue is now fixed in Q4OS Update Manager 1.6-a2.
Does this also apply to synaptic? I've received the "big" upgrade two days in a row now. Yesterday, it was 90 packages, today it's 120. The odd thing that happened today is that "reload" did not clear the list. I had to reboot to clear the 120 upgrades.

No, it is fixed for Q4OS update manager and other Q4OS specific tools. The only way to completely get rid of it, is to disable Debian Backports repositories in '/etc/apt/sources.list...'

Yes, we can confirm Synaptic sometimes doesn't refresh lists completely. The reliable way to clear it completely is to run 'apt-get update' in terminal:
$ sudo apt-get update

Or if you have my updater installed you can right click the icon and select Refresh apt db Now and it will do it for you.
