I'm trying to install q4os 5.8-x64, but it keep crashing.

Take a look at this, https://wiki.debian.org/HP/Proliant
The link isnt working properly so when you get to the site, go to search, type in hp proliant and when the list comes up pick the first listing and it will take you to the page.
There are a number of settings youll need to look at.

Its been awhile with servers this old, but many times the raid controller isnt recognized(issues with the cciss driver). Maybe disabling hardware raid if you havent done that already. Sometimes the lack of UEFI causes issues with Debian 12 with the older Proliant servers. The link I posted above talks to some of the more common issues found with these servers.

Yes, that looks like a hardware failure, most likely bad sectors.
