I keep losing the ability to lock my workstation via the Lock Session option or shortcut key. When I try and lock the session, absolutely nothing happens. Possibly related, my screensaver also never fires when this happens. They both stop working at the same time. What should I look for? If I reboot it starts working again.

I would suggest that a process of elimination is the way to go about this.
Assuming this is a real machine and not a VM as an i7 I would certainly look at installing a later kernel.
This is done very easily via a script provided in the OS.
1. As root you need to go to /usr/bin and edit qinst-kernel-bpo
in the 2nd line it says ....!="buster".....
Change buster to bullseye and keep the " "
Save
2. run
sudo qinst-kernel-bpo
Try that for a couple of days an see if it makes a difference.
If it's still going wrong consider:-
Is there any particular prog that you are leaving running when this happens?
Make sure any Autostart stuff like Conky or other things you may have introduced are off.
In Screensaver Settings>Advanced make sure all corners are No Action.
In Screensaver Settings disable 'Require Password to Stop'
Set the screensaver as Blank Screen.
Set the timer to 3 minutes.
In TDEPowersave Select 'Performance' and make sure all Tabs and Options are Off/unchecked
Shut down - not reboot - Shut down. Wait 30 sec. Start up.
Log in and open nothing.
Wait for screensaver to kick in and if that works OK and responds to mouse.
I assume you are using the Logout Applet to provide the Lock Session so try that and see if it behaves as expected.
If all is OK then keep working until it goes wrong.
At that point it's a question of whether the keyboard does anything at all. If it does then we'll need to get onto a terminal screen and take a look at some log files. To make that easier I suggest you run
sudo usermod -a -G adm
yourusername before to swoff/on

It is an i7, albeit from 2013. Also, I'm running Trinity (TDE) and not Plasma. I read that might make a difference in which kernel I can choose. Everything else on the desktop works fine, so I'm not concerned about running the "latest and greatest" kernel and I chose Q4OS because of TDE.
What application is responsible for locking the session? Maybe that app is dying and I can just re-spawn it :-)
Oh, I saw another clue, I'll try and capture a picture of it the next time this happens . . . upon rebooting, just before the GUI is closed out, a little dialog in the middle of the screen pops up (no window decorations on it, no interactive buttons to press) and says something like "The screen could not be locked because . . . ." and closes before I could read the rest.
I'll keep testing and post results here.
